The 50-year-old took over in December on an 18-month deal after the sacking of Paulo Fonseca.
Though he made an immediate impact by guiding Milan to the Italian Super Cup title just a week into his tenure, the team failed to build on that success.
Milan lost the Coppa Italia final to Bologna and struggled in the league, missing out on European qualification altogether.
Meanwhile, city rivals Inter are preparing for the Champions League final against Paris Saint-Germain this weekend, deepening Milan’s frustration.
Conceicao arrived with a strong reputation after winning 11 trophies in six seasons at Porto.
However, Milan opted to part ways, with the club thanking him and his staff for their professionalism and contribution to the club’s 50th major trophy.
Former manager Massimiliano Allegri, who led Milan between 2010 and 2014, is being lined up for a potential return.
